Lawful basis for processing the information

General Data Protection Regulation Article 6(1)(e) – processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller. 

Our additional legal basis for using your special category information is: 

Processing is necessary for the purposes of preventive or occupational medicine, for the assessment of the working capacity of the employee, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems and services on the basis of domestic law or pursuant to contract with a health professional and subject to the conditions and safeguards.

How we collect information

Personal data is collected from you through either a digital or paper form, telephone or referral service by our service provider, Solutions 4 Health.

What information is collected

The type of information collected includes, name, surname, address including postcode, telephone number(s), date of birth, age, gender, sexual orientation, ethnicity, exemption status from prescription charges, pregnancy status, occupation, housing status, select medical history (current or prior contact with mental health or drug and alcohol services, long-term condition or cancer), notes detailing Service delivery data including pharmacotherapy (where applicable). 

How we use the information provided

RBWM and our service partners, Solutions 4 Health, will use your data to provide you with the Stop Smoking Service that you have asked for or that you have been referred for.

Who has access to the information about you

Our service provider, Solutions 4 Health, will have access to your information.

Who we may share your information with

We will share your information with our service provider, Solutions 4 Health, who will be providing the Stop Smoking Service on behalf of RBWM. 

Should you have been referred to us by the voluntary NHS referral scheme, we will provide the NHS with statistical data in order to allow them to track the uptake of their programme. 

How long we store your information

Your information is stored for seven years from the last time you accessed the clinical services.
