Adult social care aims to help people stay independent, safe and well so they can live the lives they want to.
This includes:
- older people;
- people with physical disabilities;
- people with autism;
- people with learning disabilities;
- people who have mental health issues;
- people who look after family or friends;
- carers.

There are many ways that you can get support with your needs, and a conversation with adult social care can help you understand what help is available.
For some, this will be advice and signposting to help connect you to services which aim to maintain or improve your independence, equipment/aids and adaptations or an assessment by a practitioner where we will aim to understand how well you are managing everyday tasks like washing, dressing and cooking which may lead to ongoing care and support.

Adult social services referral form
If you are in recovering following an illness or injury, our Short Term Support and Reablement (STS&R) Service can help you regain stability and independence.

Social workers and practitioners
Our social workers and social care practitioners will meet with you and your carers to discuss your needs and wishes. They will support carry out your needs assessment, put in place your support plan and review your on-going needs.
Occupational therapists
Our occupational therapists and occupational therapy assistants will provide you with advice and guidance on how to prevent falls, equipment that you can use to stay independent and support to apply for adaptions around your home.
